The government's Long-term Care 2.0 initiative was launched on January 1st, including dementia patients above the age of 50 in the government long-term care program for the first time. But some doctors are criticizing the program for lacking a comprehensive case management system for dementia patients. They suggest that the government implement a community care model similar to that for diabetes patients in order to reduce the costs to society and the National Health Insurance system.
There are about 260,000 dementia patients in Taiwan, but only 70,000 are certified as disabled. Many cases are hidden and undiagnosed. Some doctors are advocating for changes to current rules limiting the diagnosis and treatment of dementia to neurologists and psychiatrists. They say that family doctors should also receive the training to discover and treat dementia.

Some people go to the same clinic since their childhood, so the doctor at that clinic has the most complete history for that patient. Here at the hospital, we need a lot more time to really get to know that patient compared to the family doctor at the clinic.
Other doctors are criticizing the government's Long-term Care 2.0 system for covering dementia patients over the age of 50, but without a comprehensive case management system. They say that case managers for patients can help with referrals, non-medical treatments, and counseling for family members. They can also reduce the frequency of ER visits due to emotional outbursts.

Elderly dementia patients often experience delusions, hallucinations, and anxiety. There's nothing that an ER can do about any of that. The most important thing is for the patient to get in touch with their doctor, who knows how to calm the patient down. Then an ER visit can be avoided.
Doctors suggest that the community care model for diabetics can be adapted for dementia patients. They also say that a case management system that provides education to family members can reduce the cost to society and the national health insurance system.
