Human error caused transasia flight crash / 基隆河復興航空事故 機師誤關引擎釀禍

The final investigation report into the crash of TransAsia Airways flight GE235 on February 4 last year was released yesterday. The report confirms that the crash was caused by human error in which the pilot retarded the aircraft's only good engine. On February 4 last year, TransAsia Airways flight GE235 crashed into Keelung River nearly three minutes after takeoff, killing 43 and injuring 15 of the 58 on board. The Aviation Safety Council released the final investigation report on Thursday, confirming that the crash was caused by a human error. ==THOMAS WANG, Aviation Safety Council Executive Director== Having looked at past training records, we discovered that Pilot A had problems dealing with abnormal situations and flight operations. This means that he should have received further training and reviews to pass certain examinations, but TransAsia Airways overlooked this type of aviation safety risk and did nothing. According to the ASC, the flight crew did not perform the documented abnormal and emergency procedures to identify the failure and implement the required corrective actions. This led the pilot flying to retard power of the operative engine number 1 and shut it down ultimately. The report also pointed out that there were weld defects in the aircraft's engines. ==THOMAS WANG, Aviation Safety Council Executive Director== The manufacturer had been aware of this defect for a long time, so all users were informed to undergo checks every 12,000 hours, but the AFU tests on engine No.2 and No.3 showed that they were only used for more than 1,000 hours. After the release of ASC's final investigation report, TransAsia Airways said that they were willing to take in criticisms and suggestions. As of the end of this June, TransAsia Airway has reached settlements with the affected crew members' families, more than 70% of the families who have lost their loved ones and half of those injured because of the crash. They are still under negotiations with the rest. TransAsia Airways offered NT$14.9 million in compensation to the family of each of the dead. TRANSLATED BY:LUKE CHIANG 去年2月4號,復興航空GE235班機,才剛起飛2分56秒,就在南港墜入基隆河,當時一共造成43人罹難、15人受傷,飛安會今天公布調查最終報告,確認是人為疏失所造成。 ==飛安會執行長 王興中== 看了正駕駛員A 他過去的一些 相關的訓練記錄 我們發現他對於 不正常的程序的操作 或發生動機的處理 一直處在一些有問題 就是需要經過 重新的訓練跟考核 才可以通過相關的考驗 但是復興(航空)並沒有針對 這一個飛安風險 在事前了解並解決這個問題 飛安會表示,飛航組員當時沒有依照執行手冊規範以及緊急程序,來辨識處理故障狀況,也沒有依程序執行改正措施,導致操控駕駛員誤收回關掉仍正常運作中的一號發動機,另外調查也發現,這架飛機發動機的焊接點出現裂痕,焊接點出現瑕疵。 ==飛安會執行長 王興中== 製造廠商已經知道(焊接點瑕疵) 這個現象 很長的一段時間 所以通知所有使用者 要在每1萬2千個小時 要去檢查一下 但是我們所測試2號跟3號的 自動順槳單元的測試 事實上它們都只用了 一千多個小時 就發生這樣的狀況 對於飛安會的最終調查,復興航空表示會虛心接受檢討,截至今年6月底,復興航空已跟超過7成的罹難者家屬達成和解,傷者跟地面受傷者有一半和解,至於還沒有和解的部分,還在積極溝通協調中,而罹難者每位則是賠償新臺幣1490萬元。
